Panther Premier
Plan members may go to Titusville Hospital for all testing and
admission. Call Member Provider
Services for notification of admission
to the hospital. Members must pay
full cost for services until deductible
is met, after which co-insurance of 10%,
for any medical service, is members
responsibility.

determined by the drug name which will
fall within the categories of generic,
preferred, non-preferred
