Important Forms

Dear Student,

On behalf of the Student Affairs staff, I’d like to welcome you to the University of Pittsburgh at Titusville. We’re looking forward to having you on campus next semester. I’m sure you have questions about your options and you’re eager to complete your housing arrangements. The following information should provide answers to your questions and explain what you should do next.

If you will be a full-time student (12 credits or more) you are required to live on campus unless you plan to commute from the home of a parent or relative. Students 21 years of age and older are excluded from this policy. Upon request, exemptions may be granted for other reasons subject to approval by the Vice President for Student Affairs. All students requesting exemption should complete and return the residency verification form stating the basis for exemption (i.e., where and with whom you will reside while attending UPT). Housing is not provided for part-time students. If you are requesting a specific roommate, both of you need to list each other in writing as your roommate preference on your housing application. Roommate requests will only be accepted until June 1st. After June 1st, roommate requests will be honored as space permits. Students have the option for room changes beginning with the third week of classes.

All students must complete the following steps:

  1. Carefully review all materials.

  2. Complete and sign ALL appropriate forms. You will not be given a housing assignment until ALL of your forms are completed and returned. These include the Housing Application, Room & Board Contract, Residency Verification Form, Meal Plan, and Health Evaluation with required immunizations. Nursing students will need to submit two different health evaluations: one for nursing and one for housing.

  3. If you are requesting housing, please return your housing forms with a $100.00 (nonrefundable) housing deposit. If you previously paid your $100.00 housing deposit for the upcoming term, then returning your housing forms is all that is required.

Should you have any questions or need additional information, please don’t hesitate to contact me at (814) 827-4460 or uptstu@pitt.edu.

Sincerely,

Scott Horch
