ALL questions from faculty, staff, or doctors regarding the UPMC Health Plan (cards, pharmacy questions, benefit claims, changing a PCP, etc.) should be directed to the UPMC Member Provider Service. When a doctor's office calls to inquire about coverage, the doctor must be specific and ask about the Titusville Campus coverage.

Express Scripts, Inc. processes all UPMC Health Plan mail service prescription program. Prescription must be sent in a Express Scripts, Inc. participant profile envelope along with your written prescription. Envelopes can be obtained from the Office of Human Resources, Bennett Davis Hall.

A University of Pittsburgh Human Resources Benefits Specialist should be called if you have questions about the overall benefits package provided by the University or about benefits not involving the UPMC Health Plan (such as life insurance, flexible spending account, etc.).

Change in Family Status

To process a change in family status for benefits purposes (new born, adoption, cancellation of coverage), you must complete a new benefit Enrollment Form as well as a Change in Family Status form. The change will supersede previous benefit information and must be accompanied by supporting documents (birth certificate, marriage certificate, etc.) All forms may be printed from the Office of Human Resources - Benefits web site at www.hr.pitt.edu.
